The practice provides the following
services:
ASTHMA
Asthmatic patients should have
regular checks and we encourage
patients to be actively involved in
their own care. We offer advice and
monitoring as well as access to
specialist services. Asthma clinics
are held most afternoons by
appointment with the practice nurse.
CORONARY HEART DISEASE
Patients at risk of coronary heart
disease should have regular checks
and we encourage patients to be
actively involved in their own care.
Our nurses offer monitoring and
advice. You will be called
automatically for an annual review,
which will be held at the surgery.
However, you should call the doctor
or nurse immediately if you have any
concerns. CVD RISK ASSESSMENT
If you have hypertension and are
aged between 40 and 74 years you may
book an appointment with our
practice nurse for a Cardiovascular
Risk Assessment. This simple
assessment will ascertain your risk
of developing Coronary Heart Disease
over a 10 year period. Please make
an appointment with the practice
nurse.
DIABETES
Diabetic patients should have
regular checks, which will be with
their doctor, nurse or a joint
appointment with them both. A
specialist diabetic nursing team is
also available for our patients.

Annual diabetic retinal screening is
now offered at Basildon and
Brentwood Community Hospital. Staff there will
contact patients direct with
appointment details. Patients are
called automatically for an annual
diabetic review, which is held here
at the surgery. However, they should
contact the surgery immediately if
they have any concerns.
MINOR OPERATIONS
Minor surgical procedures can be
arranged at the surgery after an
initial consultation with your
doctor. Examples include removal of
cysts, warts, skin lumps and the
injection of joints.
