For the first week of illness, you
should fill in a ‘self certificate’.
- If you are employed, you
need form SC2 which you can get
from your employer or
downloadable

- If you are self employed,
disabled or unemployed, you need
form SC1. (Incapacity Benefit
Form). These are available at
the surgery or can be collected
from post offices and DSS
offices.
- If you want a private
certificate for the first few
days of illness these are
available but there is a charge
for them. Do you really need it?
After the first week, you will need
to contact the doctor for a further
certificate.
If you are requesting a continuation
of your sick note please call after
11.30am.
